Mortgage Application Checklist

  1. If a purchase, a copy of your sales contract with original signatures. If a refinance, bring a copy of the deed and title insurance policy.
  2. Copy of your social security card and drivers license.
  3. Name, addresses, account numbers, and phone numbers of all landlords/mortgage companies covering the last two full years
  4. Employer’s address, person to contact, and phone numbers necessary to verify any jobs all borrowers have held within the last two years
  5. A schedule showing the gross monthly income, separating base pay from commission, bonus income, and/or overtime pay for all borrowers. Bring your most recent three pay stubs.
  6. Proof of social security/disability income (i.e. award letter) if applicable
  7. Copies of Prior Year's U. S. Federal Income Tax Return, including all schedules.
  8. Account numbers/addresses/phone numbers for all checking, savings, and credit union accounts. Copy of last 2 months statements on all accounts.
  9. Most recent statement for all investments including retirement funds, stocks, and/or bonds
  10. A statement of the estimated current value of all personal and household items
  11. List of all outstanding loans and charge accounts, with address, account number, monthly payment, and balance on each (this can be listed in section VI of the loan application)
  12. If applicable, a copy of original divorce decree, settlement papers, and/or child support agreement if you pay or receive child support, alimony, or separate maintenance
  13. For self employed persons only: a year to date profit and loss statement and a current balance sheet, signed & dated by a CPA and yourself, as well as 2 years business tax returns signed and dated by you
  14. A diploma or grades transcript if the borrower was a student during the last 2 years
  15. VA loans require child care cost to be disclosed at application
  16. VA loans require DD214 (discharge papers) and original certificate of eligibility.
  17. Bankruptcy petition and discharge papers if applicable within last 7 years. Attach a letter of explanation signed and dated
  18. Investors: Copy of rental/lease agreement on rental property. Also, lender’s address, account number, phone numbers, and monthly payment amounts
  19. Copy of House Plans if this is a construction loan
  20. Cost Estimate To Build if this is a construction loan
  21. Check in the amount of $300.00 for appraisal, credit report, and application fee
